If you're a person with ALS or you care for one, we're here to help. We'll connect you with the local resources you need, including equipment loans, grants to help with ALS-related expenses, and access to ALS clinics and clinical trials.
NEW PATIENT ENROLLMENT
QUALITY OF LIFE REIMBURSEMENT PROGRAM
This year, The ALS Association Central & Southern Ohio Chapter can reimburse individuals who qualify for the Quality of Life Reimbursement Grant, individuals MUST meet the qualifying criteria--
1. Your primary residence must be within the Chapter Territory. (Please click here to view of the Chapter Territory Map)
2. You must be registered with the Chapter (To register with the Chapter, call 866.273.2572 or email - alsohio@alsohio.org
